For translation or more information on this topic, please contact author.<\/h4>\n<p>&nbsp;<\/p>\n<p>Ve V\u00daV TGM, v. v. i., je od dubna 2020 prov\u00e1d\u011bn monitoring p\u0159\u00edtomnosti koronaviru SARS-CoV-2 v\u00a0odpadn\u00edch vod\u00e1ch. Metoda pr\u016fkazu je obdobn\u00e1 jako u\u00a0klinick\u00fdch vzork\u016f, tj. pomoc\u00ed PCR je zji\u0161\u0165ov\u00e1na p\u0159\u00edtomnost specifick\u00e9ho fragmentu virov\u00e9 RNA. V\u00fdznamn\u00fd rozd\u00edl je v\u00a0n\u00e1ro\u010dnosti \u00fapravy vzorku odpadn\u00ed vody, kdy je z\u00a0n\u00ed nutn\u00e9 separovat \u010distou RNA s\u00a0minimalizac\u00ed ztr\u00e1t jej\u00edho c\u00edlov\u00e9ho mno\u017estv\u00ed. Anal\u00fdzy prov\u00e1d\u00ed virologick\u00e1 laborato\u0159 V\u00fdzkumn\u00e9ho \u00fastavu veterin\u00e1rn\u00edho l\u00e9ka\u0159stv\u00ed (V\u00daVeL) v\u00a0Brn\u011b, kter\u00e1 m\u00e1 s\u00a0detekc\u00ed vir\u016f v\u00a0odpadn\u00edch vod\u00e1ch dlouholet\u00e9 zku\u0161enosti. Stejn\u00fd monitoring na vybran\u00fdch \u010cOV byl prov\u00e1d\u011bn v\u00a0jarn\u00edm obdob\u00ed spole\u010dnost\u00ed SOVAK, kter\u00e1 zji\u0161t\u011bn\u00e1 data poskytla do spole\u010dn\u00e9ho hodnocen\u00ed. Vyhodnocen\u00ed je prov\u00e1d\u011bno ve spolupr\u00e1ci se St\u00e1tn\u00edm zdravotn\u00edm \u00fastavem (SZ\u00da), kter\u00fd ze zdravotnick\u00e9ho sektoru zajistil \u00fadaje o\u00a0po\u010dtech infikovan\u00fdch osob v\u00a0monitorovan\u00fdch oblastech. V\u00a0prvn\u00edm kole monitoringu bylo odebr\u00e1no celkem 137 vzork\u016f n\u00e1tok\u016f na 40 \u010dist\u00edrn\u00e1ch odpadn\u00edch vod (\u010cOV) z\u00a0cel\u00e9ho \u00fazem\u00ed \u010cR. Pozitivn\u00ed n\u00e1lez specifick\u00e9ho fragmentu viru SARS-CoV-2 byl zji\u0161t\u011bn u\u00a028 % \u010cOV v\u00a015 % vzork\u016f.<\/p>\n<p>V\u00a0sou\u010dasn\u00e9 dob\u011b prob\u00edh\u00e1 vyhodnocen\u00ed jarn\u00edho kola odb\u011br\u016f s\u00a0vyu\u017eit\u00edm \u00fadaj\u016f o\u00a0po\u010dtech infikovan\u00fdch osob v\u00a0konkr\u00e9tn\u00edch oblastech, kter\u00e9 byly v\u00a0pr\u016fb\u011bhu monitoringu nedostupn\u00e9. Data o\u00a0osob\u00e1ch pozitivn\u00edch na SARS-CoV-2 jsou z\u00edsk\u00e1na z\u00a0celost\u00e1tn\u00edho syst\u00e9mu ISIN (Informa\u010dn\u00ed syst\u00e9m infek\u010dn\u00edch nemoc\u00ed). Data do ISIN jsou vkl\u00e1d\u00e1na z\u00a0Krajsk\u00fdch hygienick\u00fdch stanic. Ke grafick\u00e9mu zobrazen\u00ed epidemiologick\u00e9 situace bylo vyu\u017eito datum prvn\u00edch p\u0159\u00edznak\u016f a\u00a0p\u0159edpoklad, \u017ee \u010dlov\u011bk je infek\u010dn\u00ed a\u017e 3 dny p\u0159ed t\u00edmto datem a\u00a0nejv\u00edce viru vylu\u010duje do 10. dne od prvn\u00edch p\u0159\u00edznak\u016f. U\u00a0n\u011bkter\u00fdch osob vylu\u010dov\u00e1n\u00ed prob\u00edh\u00e1 d\u00e9le ne\u017e 10\u00a0dn\u00ed, co\u017e je v\u00a0grafech zobrazeno slab\u0161\u00ed barvou. D\u00e9lka se odv\u00edj\u00ed od toho, zda je zn\u00e1mo datum negativn\u00edho PCR testu, pokud ne, je \u0159ada ukon\u010dena po 30 dnech. Pokud osoba zem\u0159ela, \u0159ada kon\u010d\u00ed datem \u00famrt\u00ed. Grafy zobrazuj\u00ed v\u00fdsledky monitoringu odpadn\u00edch vod v\u00a0konkr\u00e9tn\u00edch datech.<\/p>\n<p>Monitoring pokra\u010duje podzimn\u00edm kolem odb\u011br\u016f vzork\u016f odpadn\u00edch vod, aby byly z\u00edsk\u00e1ny informace z\u00a0druh\u00e9 vlny epidemie, kdy jsou po\u010dty infikovan\u00fdch osob \u0159\u00e1dov\u011b vy\u0161\u0161\u00ed ne\u017e v\u00a0jarn\u00edm obdob\u00ed. Pod\u00edl infikovan\u00fdch osob na po\u010dtu obyvatel napojen\u00fdch na sledovan\u00e9 \u010cOV se na ja\u0159e pohyboval kolem 0,1 %, v\u00a0podzimn\u00ed vln\u011b je to cca 2\u00a0%.<\/p>\n<p>Za \u00fa\u010delem zachycen\u00ed n\u00e1stupu dal\u0161\u00ed vlny zv\u00fd\u0161en\u00e9ho v\u00fdskytu onemocn\u011bn\u00ed COVID-19 byla ze 40 sledovan\u00fdch \u010cOV vybr\u00e1na jedna st\u0159edn\u011b velk\u00e1 \u010cOV, na n\u00ed\u017e je od dubna kontinu\u00e1ln\u011b prov\u00e1d\u011bn monitoring s\u00a0frekvenc\u00ed odb\u011br\u016f 1\u20132 t\u00fddny. Na t\u00e9to \u010cOV byla p\u0159\u00edtomnost virov\u00fdch fragment\u016f v\u00a0jarn\u00edm kole monitoringu prok\u00e1z\u00e1na ve 3 z\u00a08 odb\u011br\u016f. Z\u00e1chyt odpov\u00eddal 3\u201325 evidovan\u00fdm infikovan\u00fdm osob\u00e1m z\u00a0v\u00edce jak 20\u00a0000 napojen\u00fdch obyvatel. V\u00a0oblasti napojen\u00e9 na tuto \u010cOV bylo v\u00a0dob\u011b prvn\u00ed vlny (do 7. 7. 2020) evidov\u00e1no cca 60 infikovan\u00fdch osob, tj. cca\u00a00,3\u00a0% napojen\u00fdch obyvatel. V\u00a0podzimn\u00ed vln\u011b zde po\u010det infikovan\u00fdch osob stoupl na cca 2\u00a0% (&gt; 400 osob). Pozitivn\u00ed n\u00e1lez virov\u00e9 RNA v\u00a0odpadn\u00ed vod\u011b zde od jarn\u00ed vlny nebyl zachycen. P\u0159\u00ed\u010dinou m\u016f\u017ee b\u00fdt rozd\u00edl v\u00a0pr\u016fb\u011bhu nemoci v\u00a0jarn\u00ed a\u00a0podzimn\u00ed vln\u011b, a\u00a0tedy s\u00a0mo\u017en\u00fdm men\u0161\u00edm mno\u017estv\u00edm virov\u00fdch \u010d\u00e1stic, obsa\u017een\u00fdch ve stolici nebo mo\u010di v\u00a0lokalit\u011b s\u00a0intenzivn\u00edm v\u00fdskytem v\u00a0jarn\u00ed vln\u011b.<\/p>\n<p>Dosavadn\u00ed v\u00fdsledky v\u00fdzkumu n\u00e1m potvrdily informace publikovan\u00e9, na za\u010d\u00e1tku pandemie, nizozemsk\u00fdmi v\u00fdzkumn\u00edky o\u00a0v\u00fdskytu virov\u00fdch fragment\u016f viru SARS-CoV-2 v\u00a0ne\u010di\u0161t\u011bn\u00fdch odpadn\u00edch vod\u00e1ch. Jak vypl\u00fdv\u00e1 z\u00a0klinick\u00fdch studi\u00ed, je jejich pravd\u011bpodobn\u00fdm zdrojem tr\u00e1vic\u00ed a\u00a0vylu\u010dovac\u00ed syst\u00e9m \u010dlov\u011bka. T\u00e9ma p\u0159\u00edtomnosti koronavir\u016f ve st\u0159evech a\u00a0mo\u010di v\u0161ak nen\u00ed dosud dostate\u010dn\u011b klinicky prob\u00e1d\u00e1no, ale v\u00fdzkum intenzivn\u011b pokra\u010duje. V\u00fdznamn\u00fd objev ud\u011blali v\u011bdci z\u00a0Hubrechtova institutu v\u00a0Utrechtu, kte\u0159\u00ed v\u00fdzkumy na tk\u00e1\u0148ov\u00fdch kultur\u00e1ch potvrdili, \u017ee viry SARS-CoV-2 mohou infikovat bu\u0148ky st\u0159ev a\u00a0mno\u017eit se v\u00a0nich.\u2028T\u00edmto poznatkem se v\u00fdznamn\u011b zvy\u0161uje v\u00fdznam monitoringu odpadn\u00edch vod a\u00a0studium mo\u017en\u00e9 kontaminace vodn\u00edho prost\u0159ed\u00ed. Pr\u016fkaz p\u0159enosu fek\u00e1ln\u011b-or\u00e1ln\u00ed cestou v\u0161ak dosud prok\u00e1z\u00e1n nebyl. Zat\u00edm nejsou zn\u00e1my \u00fadaje, zda se v\u00a0t\u011bln\u00edch exkretech vyskytuj\u00ed fragmenty viru u\u00a0v\u0161ech infikovan\u00fdch osob nebo zda z\u00e1le\u017e\u00ed na pr\u016fb\u011bhu a\u00a0symptomech, v\u00a0jak\u00e9m mno\u017estv\u00ed, po jak dlouhou dobu, v\u00a0kter\u00e9 f\u00e1zi nemoci a\u00a0zda i\u00a0u\u00a0bezp\u0159\u00edznakov\u00fdch jedinc\u016f. D\u016fle\u017eit\u00e1 je tak\u00e9 konkr\u00e9tn\u00ed imunitn\u00ed odpov\u011b\u010f infikovan\u00e9 osoby, kter\u00e1 m\u016f\u017ee prob\u00edhat r\u016fzn\u00fdmi mechanismy, z\u00a0nich\u017e n\u011bkter\u00e9 mohou genom viru rozlo\u017eit a\u017e na nedetekovateln\u00e9 fragmenty. Lze tedy p\u0159edpokl\u00e1dat, \u017ee neexistuje p\u0159\u00edm\u00e1 \u00fam\u011bra mezi po\u010dtem infikovan\u00fdch osob a\u00a0kontaminac\u00ed odpadn\u00edch vod virov\u00fdmi fragmenty.<\/p>\n<p>V\u00fdsledky prvn\u00ed vlny monitoringu p\u0159inesly, d\u00edky celkov\u011b velmi n\u00edzk\u00e9mu po\u010dtu infikovan\u00fdch osob v \u010cR v prvn\u00ed vln\u011b epidemie, jedine\u010dn\u00e9 v\u00fdsledky, nebo\u0165 umo\u017enily zachytit doln\u00ed mez detekce zvolen\u00e9 metody. Pozitivn\u00ed n\u00e1lez fragment\u016f SARS-CoV-2 byl stanoven u \u010cOV, kde byly aktu\u00e1ln\u011b v dan\u00fd den evidov\u00e1ny 1\u20133 infikovan\u00e9 osoby z objemu odpadn\u00edch vod od 6 000 do 12 000 napojen\u00fdch obyvatel. Vzhledem k p\u0159edpokl\u00e1dan\u00e9mu \u010dasov\u00e9mu intervalu mezi hl\u00e1\u0161en\u00edm infekce a\u00a0potenci\u00e1ln\u00edm vylu\u010dov\u00e1n\u00edm vir\u016f do odpadn\u00edch vod, je zde vhodn\u011bj\u0161\u00ed jako mo\u017en\u00e9 maximum pou\u017e\u00edt kumulativn\u00ed \u00fadaj o\u00a0po\u010dtu infikovan\u00fdch osob v\u00a0dan\u00e9 lokalit\u011b za cel\u00e9 jarn\u00ed obdob\u00ed (do 7. 7. 2020). Tak\u00e9 v\u00a0tomto p\u0159\u00edpad\u011b byly v\u00a0odpadn\u00edch vod\u00e1ch zachyceny fragmenty virov\u00e9 RNA ji\u017e od 3\u20135 infikovan\u00fdch osob z\u00a0objemu odpadn\u00edch vod od 6\u00a0000 do 80\u00a0000 napojen\u00fdch obyvatel. Tato velmi n\u00edzk\u00e1 \u010d\u00edsla ukazuj\u00ed na velmi vysokou citlivost metody. P\u0159\u00edklad \u010cOV s\u00a0negativn\u00edm n\u00e1lezem viru SARS-CoV-2 v\u00a0odpadn\u00ed vod\u011b a\u00a0pozitivn\u00edm v\u00fdskytem infikovan\u00fdch osob<\/h6>\n<p>Prvn\u00edm faktem je zji\u0161t\u011bn\u00e1 absence viru v\u00a0odpadn\u00edch vod\u00e1ch v\u00a0lokalit\u00e1ch, kde byl evidov\u00e1n v\u00fdskyt pozitivn\u011b testovan\u00fdch osob (<em>obr. 1<\/em>). P\u0159\u00ed\u010dinou m\u016f\u017ee b\u00fdt velk\u00e9 na\u0159ed\u011bn\u00ed odpadn\u00edch vod, tj. hledan\u00e1 \u010d\u00e1stice genomu nebyla do vzorku zachycena v\u00a0detekovateln\u00e9m mno\u017estv\u00ed nebo stavu (do\u0161lo nap\u0159. k\u00a0degradaci RNA pomoc\u00ed enzym\u016f \u0161t\u011bp\u00edc\u00edch RNA ji\u017e p\u0159ed odb\u011brem nebo p\u0159i transportu vzorku). Dal\u0161\u00edm d\u016fvodem m\u016f\u017ee b\u00fdt fakt, \u017ee se virov\u00e9 \u010d\u00e1stice nedostaly do tr\u00e1vic\u00edho nebo vylu\u010dovac\u00edho syst\u00e9mu infikovan\u00fdch osob, jak bylo pops\u00e1no v\u00fd\u0161e, nebo \u017ee se v\u00a0dob\u011b nemoci tyto osoby nevyskytovaly v\u00a0m\u00edst\u011b, kde byly evidov\u00e1ny jako infikovan\u00e9 (pobyt v\u00a0zam\u011bstn\u00e1n\u00ed, v\u00a0rekrea\u010dn\u00edch objektech apod.).<\/p>\n<a href=\"https:\/\/www.vtei.cz\/wp-content\/uploads\/2021\/02\/Mlejnkova-2.jpg\" rel=\"shadowbox[sbpost-11142];player=img;\"><img decoding=\"async\" width=\"800\" height=\"638\" class=\"alignnone size-full wp-image-11135 lazyload\" data-src=\"https:\/\/www.vtei.cz\/wp-content\/uploads\/2021\/02\/Mlejnkova-2.jpg\" alt=\"\" data-srcset=\"https:\/\/www.vtei.cz\/wp-content\/uploads\/2021\/02\/Mlejnkova-2.jpg 800w, https:\/\/www.vtei.cz\/wp-content\/uploads\/2021\/02\/Mlejnkova-2-300x239.jpg 300w, https:\/\/www.vtei.cz\/wp-content\/uploads\/2021\/02\/Mlejnkova-2-768x612.jpg 768w\" data-sizes=\"(max-width: 800px) 100vw, 800px\" src=\"data:image\/svg+xml;base64,PHN2ZyB3aWR0aD0iMSIgaGVpZ2h0PSIxIiB4bWxucz0iaHR0cDovL3d3dy53My5vcmcvMjAwMC9zdmciPjwvc3ZnPg==\" style=\"--smush-placeholder-width: 800px; --smush-placeholder-aspect-ratio: 800\/638;\" \/><\/a>\n<h6>Obr. 2. P\u0159\u00edklad \u010cOV s\u00a0pozitivn\u00edm n\u00e1lezem viru SARS-CoV-2 v\u00a0odpadn\u00ed vod\u011b a\u00a0pozitivn\u00edm v\u00fdskytem infikovan\u00fdch osob<\/h6>\n<p>Pro v\u00fdvoj syst\u00e9mu v\u010dasn\u00e9ho varov\u00e1n\u00ed budou nejd\u016fle\u017eit\u011bj\u0161\u00ed v\u00fdsledky z\u00a0lokalit, kde byly nalezeny pozitivn\u00ed vzorky odpadn\u00edch vod a\u00a0hl\u00e1\u0161ena p\u0159\u00edtomnost pozitivn\u011b testovan\u00fdch osob (<em>obr. 3<\/em>). Pro spr\u00e1vnou interpretaci t\u011bchto v\u00fdsledk\u016f je t\u0159eba d\u016frazn\u011b akceptovat rozd\u00edly mezi typy a\u00a0charakterem \u010dist\u00edren odpadn\u00edch vod, nebo\u0165 ne v\u0161echny jsou vhodn\u00e9 pro epidemiologick\u00e9 progn\u00f3zy. \u010cist\u00edrny odpadn\u00edch vod se li\u0161\u00ed p\u0159edev\u0161\u00edm mno\u017estv\u00edm odpadn\u00edch vod, kter\u00e9 jsou na n\u011b p\u0159iv\u00e1d\u011bny; po\u010dtem napojen\u00fdch obyvatel; d\u00e9lkou a\u00a0v\u011btven\u00edm kanaliza\u010dn\u00ed s\u00edt\u011b a\u00a0ve v\u011bt\u0161in\u011b p\u0159\u00edpad\u016f zpracov\u00e1n\u00edm kombinace komun\u00e1ln\u00edch a\u00a0pr\u016fmyslov\u00fdch odpadn\u00edch vod (<em>obr. 4<\/em>). Tento c\u00edl, spolu s\u00a0optimalizac\u00ed metodiky detekce viru SARS-CoV-2 v\u00a0odpadn\u00edch vod\u00e1ch, bude v\u00a0letech 2021\u20132022 \u0159e\u0161en v\u00a0r\u00e1mci projektu \u201eVyu\u017eit\u00ed monitoringu odpadn\u00edch vod jako n\u00e1stroje v\u010dasn\u00e9ho varov\u00e1n\u00ed p\u0159ed vznikem epidemiologick\u00e9 situace\u201c, kter\u00fd byl podpo\u0159en ve 4. ve\u0159ejn\u00e9 sout\u011b\u017ei \u201eProgramu bezpe\u010dnostn\u00edho v\u00fdzkumu \u010cesk\u00e9 republiky v\u00a0letech 2015\u20132022\u201c (BV\u00a0III\/1-VS).<\/p>\n<p>Hlavn\u00edmi v\u00fdstupy projektu budou dv\u011b metodiky: \u201eMetodika vyu\u017eit\u00ed monitoringu rizikov\u00fdch biologick\u00fdch agens a\u00a0biomarker\u016f v\u00a0komun\u00e1ln\u00edch odpadn\u00edch vod\u00e1ch jako n\u00e1stroje pro v\u010dasn\u00e9 epidemiologick\u00e9 varov\u00e1n\u00ed\u201c a\u00a0\u201eMetodika detekce vybran\u00fdch rizikov\u00fdch mikrobiologick\u00fdch agens pomoc\u00ed PCR v\u00a0odpadn\u00edch vod\u00e1ch\u201c.
